After your payment is successfully processed, a confirmation page will appear indicating that your payment has been approved.

Next, you’ll need to fill out the form:

1.  After your payment is complete, you will be able to access the remaining sections of the form.
2. Fill out the form step by step.
3.  Once you have completed your entries, please print a copy on the submission page.

You can find all the important details like the date, time, and location on the screening exercise slip you printed when you registered online. It’s super important to stick to the dates on that slip, as you won’t be considered if you miss your assigned screening test.
